Earlier you stated the cause was "making bad decisions". Only stupidity (or ignorance) would fall in that category. Now you've added lack of discipline and laziness? Those might be character traits, but also things that could be improved with better education, role models, and a society that gives a damn.
This one's for Lewk: in your Texan neck 'o the woods, I'd say it takes a particular discipline and active effort to take a bus route, with transfers and walking, just to get to the motel/restaurant/crop field. Then spend at least 8 hours doing physical labor. Then take the same bus route back home, dog tired, preparing dinner and helping kids with homework. I can see how beans and rice, or beanie weanies might be on the menu. Especially if rent and utilities eat a big chunk of income.
I'd like to see YOU try and get ahead, provide for yourself and your family, making Texas minimum wage. Without using the cheaper carbohydrate or HFCS foods that can cause weight gain.

That's three, but ok. Why are poor people undisciplined, stupid and lazy? (Are they all this way? What about the ones that work but are fat? Is that discipline? But what if they work, are fat, and live in an urban food desert - that is they would have to drive 45 minutes to get to the nearest grocery store, and that store sells healthy food for double what the unhealthy food costs and they don't make that much money even though they work 55 hours a week...?)

From the book The millionaire next door, which focused on sole-proprietorship businesses. Which means you're going to get a lot of "rich in land, poor in food" type of false positives.
Even if you ignore the bad math in reaching that number, that still leaves 1 in 5 millionaires an unearned fortune.

#3. I read this book based on YOUR recommendation. YOU said it was good and suggested I would like it. I gave you something I give few people: the respect of picking up and reading a book you suggested was a good read. I should think you would be a little more appreciative of the hours of my ever dwindling time I bet on your tastes.
#4. Given that this book was suggested to me as a good read by you, one can only conclude you were prepared to discuss some of its oddities and its not-quite-clear and somewhat unhappy ending, which one could only assume you would be able to remember since you enjoyed the book so much you staked your reputation upon it by recommending I read it.
